Three years after a viral parasite called the Symbiont began fusing strangers into permanently bonded pairs, Kofi James Carter, a West Philadelphia ER doctor turned enclave medic, gets infected during a pharmacy raid when a scavenger named Rae Delgado slams into him mid-fight and her infected blood mixes into a wound torn through his double-layered gloves. Once locked together, the two strangers have to stay within a shrinking radius of each other or both die, and every mile of the walk south toward a rumored cure in Washington DC tests what the bond is turning them into. They survive a Maryland militia camp run by a colonel who studies the Symbiont as racial contamination, a Baltimore Leash Authority labor camp that collars bonded pairs into obedient drone workers, and a predatory bonded pair named Silas and Ivy who offer Rae a taste of unchecked power and try to pull her away from Kofi's morality.

An older bonded couple, Marsha and Olu, teach them that the bond runs on trust and drains on fear. Kofi breaks under Captain Miller's torture and gives up Rae's location to end his own isolation, then fights free and races back to save her from sepsis and a Leash Authority strike team, with an assist from Silas and Ivy. By the time they reach the DC wall, neither wants a cure anymore.

They choose each other, kill Silas and Ivy in a final fight, talk a teenage guard into opening the gate, and walk into the fortress meaning to tear it apart from the inside. The book ends there, mid-invasion, closing out book one of a series.
